City Business License Application

This is the Wood Village Business License and renewal form.  The full form is required for every new business, and every three (3) years for renewals (there is not a short form option this year).  Along with this form, there is an annual fee of $50.00 which may be pro-rated, plus additional fees for amusement devices.  You may pay the entire renewal fee in person at the Wood Village City Hall, or pay it online after you submit this form.  If your business makes less than $25,000 in gross revenue, you are exempt from the $50.00 fee.  However, you will need to send in a copy of your Schedule C for verification and qualification purposes.
